В статье рассматривается актуальная проблема организации комплексной помощи пациентам с сочетанными психическими, неврологическими, соматическими и дезадаптивными расстройствами. Целью работы является анализ базовых принципов формирования и эффективного функционирования мультидисциплинарной команды (МДК), включающей психиатра, психотерапевта, клинического психолога, дефектолога, невролога и терапевта. Методологическую основу составляет системный анализ и синтез данных доказательной медицины, организационной психологии и клинической практики. В результате определены ключевые принципы: иерархичность со сменным ситуационным лидерством, строгое ролевое распределение экспертиз в рамках биопсихосоциальной модели, внедрение стандартизированных протоколов междисциплинарной коммуникации и ориентация на единые функциональные цели реабилитации. Делается вывод, что синергетический эффект МДК достигается не спонтанно, а является следствием целенаправленного проектирования её структуры, процессов и общей концептуальной основы, что обеспечивает целостный и персонализированный подход к пациенту.
The article discusses the urgent problem of organizing comprehensive care for patients with combined mental, neurological, somatic and maladaptive disorders. The aim of the work is to analyze the basic principles of the formation and effective functioning of a multidisciplinary team (MDK), including a psychiatrist, a psychotherapist, a clinical psychologist, a defectologist, a neurologist and a therapist. The methodological basis is a systematic analysis and synthesis of evidence-based medicine, organizational psychology, and clinical practice data. As a result, the key principles were identified: hierarchy with replaceable situational leadership, strict role-based distribution of expertise within the framework of the biopsychosocial model, the introduction of standardized protocols for interdisciplinary communication and focus on common functional goals of rehabilitation. It is concluded that the synergetic effect of MDK is not achieved spontaneously, but is a consequence of the purposeful design of its structure, processes and general conceptual framework, which provides a holistic and personalized approach to the patient.
