В работе проведен анализ актуальной научной литературы, включающей результаты клинических исследований, мета-анализов и руководств по лечению биполярного аффективного расстройства (БАР), опубликованных в последние годы. Рассмотрены современные диагностические критерии, используемые шкалы оценки состояния, а также дифференциальная диагностика с другими психическими расстройствами. Обсуждаются современные гипотезы развития данного заболевания, в том числе генетическая, биохимическая и перинатальная. Представлен обзор современных стратегий фармакотерапии БАР, включая использование стабилизаторов настроения (лития, вальпроатов, карбамазепина), атипичных антипсихотиков и антидепрессантов. Описаны принципы индивидуального подбора медикаментозной терапии, мониторинга побочных эффектов и профилактики рецидивов. Рассмотрена роль психотерапии, в частности семейно-ориентированная терапия в комплексном лечении БАР.

The paper analyzes the current scientific literature, including the results of clinical trials, meta-analyses and guidelines for the treatment of bipolar affective disorder (BAD) published in recent years. Modern diagnostic criteria, used scales for assessing the condition, as well as differential diagnosis with other mental disorders are considered. Modern hypotheses of the development of this disease, including genetic, biochemical and perinatal, are discussed. An overview of modern pharmacotherapy strategies for BAD is presented, including the use of mood stabilizers (lithium, valproates, carbamazepine), atypical antipsychotics, and antidepressants. The principles of individual selection of drug therapy, monitoring of side effects and prevention of relapses are described. The role of psychotherapy, in particular family-oriented therapy in the complex treatment of BAD is considered.
